Please note that you need to consult with a parent or guardian while filling out this portion of the application form as it pertains to your availability and capability to be an active and present member of the Discovery Center Young Leaders Council as well as waivers and photo releases should you be selected as a 2025 - 2026 Young Leaders Council member.
By agreeing this waiver I confirm that my child will be available and have adequate transportation to be a present and active member of the Discovery Center Young Leaders Council. The Discovery Center Young Leaders Council will meet twice monthly for two (2) hours, for a total of four (4) hours per month. The Discovery Center Young Leaders Council will convene from September 2025 - April 2026.
Additional opportunities including, but not limited to Discovery Center Mentor Sessions, Young Leaders Council Project Sessions and additional special event volunteer opportunities may be available to Young Leaders Council members. Additional Young Leaders Council opportunities are encouraged, but not required to be a member of the Discovery Center Young Leaders Council.
I acknowledge that unnecessarily missing Young Leaders Council meetings and events may hinder my students capability to actively participate and receive the full benefits of membership with Discovery Center’s Young Leaders Council Program.
If my student has to miss for any reason I will encourage them to reach out to program coordinator, Rachel Hooper at rhooper@explorethedcorg.dreamhosters.com.
I do hereby voluntarily agree to appear and/or have my child(ren) appear in the photographs and videos taken at Discovery Center.
I understand that those photographs and videos will be used on or in publications or promotional materials associated with Discovery Center at Murfree Spring. I hereby assign all rights for the usage of my photographs and videos to Discovery Center at Murfree Spring, its associates and partners. No further claim of any kind will be made against Discovery Center at Murfree Spring, its associations or partners in regards to this photography and video contract.
As a student I confirm that by applying to join Discovery Center’s Young Leaders Council program I will make a reasonable effort to attend and participate in Young Leaders Council bi-weekly meetings. I acknowledge that my attendance and participation will allow me to receive the full benefits of being a Discovery Center Young Leaders Council Member as well as support my fellow members. If selected as Discovery Center Young Leaders Council Member I will adhere to the attendance guidelines set in the Discovery Center Young Leaders Council guidelines and bylaws.
